Data Protection Legislation allows service users or staff to find out what information we hold about them and this includes health records, staff records, etc.
Please submit the information request via our secure SAR Portal in order for you to receive the information requested securely.Â
You may need to register if you do not already have an account on the portal. Once complete, you will be able to view your request/records securely using a download function within the portal, reducing the need for paper copies, DVD files, etc.
Information you request under a subject access request is supplied free of charge.
If you make a request for further copies of the same information we may charge a reasonable fee to cover the cost of supplying this. The fee will be based on the administrative cost of providing the information and will be advised to you at the time of your request if this is relevant.
Once we have received your application and consent form, we will respond within one month to provide the information you have requested. Very occasionally it may not be possible to comply within this time frame, but you will be informed if this is the case within one month of receipt of your request to explain why the extension is necessary. In order to avoid a delay, please specify the nature of the information you require on the application form.
 If you feel that the information that we hold about you is incomplete or incorrect please let us know by submitting a request through the SAR portal.
